Country Meadows Clinic will be under the ownership and management of Impact Family Wellness, a healthcare provider who delivers high-quality care through a membership-based model called Direct Primary Care, starting March 1.
The clinic is moving from an insurance-based program into a membership model with unlimited access to their providers with no copay charges.
There will be limited spots available with a cap of 500 patients. Registering before March 1 will allow patients to have the enrollment fee waived.
Memberships are $79 to $95 per month for adults, and $45 per month for children.
Patients will experience unhurried appointments and personalized attention from their providers as well as transparent pricing with no hidden fees. Same day or next day appointments are available as well as telemedicine options and direct communication with their healthcare team. Patients will also have the convenience of texting their provider for immediate, personalized service.
