Athletic physicals for the 2022-2023 school year

Parents and Athletes, As the spring semester comes to an end, it is time for all students planning to participate in athletics for the 2022-23 school year to update their paperwork.

Incoming 7th, 9th and 11th grades are required to have a physical. Physicals will be offered to all High School and Junior High athletes on Thursday, April 28, 2022 at the High School from 1:00 pm until 5:00 pm, for $10. Cash Only.

In order to receive a physical on April 28th the athlete will need to turn in a completed Medical History Form and $10. Turn completed forms and money in to:

• Coach Morgan Holly- Jr. High girls

• Coach Eric Willard- Jr. High boys

• Coach Beaver- High School athletes

The deadline is Wednesday, April 27th . Athletes that wear glasses or contacts will need to bring them with so they can complete an eye exam during their physical. If the athlete has any other medical condition (i.e. - asthma, diabetes, taking medications, etc.) please bring documentation from the athlete's doctor if possible. The athlete may not pass their physical and need further evaluation if no documentation is provided.

Athletes that do not participate in this opportunity will need to take the Medical History and Physical forms to a physician to be cleared for participation. **Incoming 7th, 9th, 11th grades are required to get a physical for 2022-23 school year.** The online paperwork can be found at https://tinyurl.com/y4pnjb42. Everyone who plans participate in athletics during the 2022-23 school year will need to complete these forms before they are eligible to practice or play.
